As an experienced Care Coordinator based with Derbyshire Dales Primary Care Network (Matlock, DE4), you will deliver practical, person-centred support for patients with complex health and social needs. The role combines operational and administrative responsibilities: coordinating the PCN Home Visiting Team, managing referrals into the Home from Hospital / Transfer & Settle pathway, organising appointments, attending MDT meetings, maintaining accurate clinical records (GDPR-compliant) and providing cover across practices to ensure continuity of care. The post involves some travel across the Derbyshire Dales, occasional partner-site meetings and a weekend rota (Saturday and Sunday 08:00–18:00 as part of a five-day rota); job share and flexible working can be considered to help balance commitments.

This opportunity is ideal for experienced carers or staff with a background in health or social care who are organised, confident with digital systems (Microsoft Office) and sensitive handling confidential information. You’ll join a friendly, supportive team working across 11 GP practices, with full training and supervision, free on-site parking at the main office (Scholes Mill, Tansley) and the chance to shape joined-up care in local communities. A GCSE in English and Maths, a full clean driving licence and a satisfactory DBS check are required.
