Role Profile & ResponsibilitiesProvide ongoingaftercare support and interventions that helpsustain the goals andprogress of those exiting in-depthpsychosocial interventions (via Active Treatment)Main Duties:CaseworkEngage in joint reviewsand proactive liaisonto ensure a smoothhandover into Aftercare fromother GDAS treatment strands or Tier 4 services.Via 1:1 or group setting, providethe following for an extensiveAftercare cohort:Createand co-ordinate dynamic, holistic Recovery Plans, encouraging engagement with aftercare servicesfor a minimum of seven months post ActiveTreatmentPost detox and/or aftercareservices for drug and alcohol service usersDeliver Life Skills, BreakingFree, CRA, ITEP,TCU and otherAftercare focussedinterventions and programmesConduct Treatment Outcome Profiles (TOPS)Attendmedical assessments and reviews with the GDAS medical team (where necessary), and provide specificresponses to formerusers,
