Part‑time 18 hours/week — ideal for balancing work with family or caring responsibilities.
Fixed‑term 2 years with strong chance to extend or move into a permanent role.
Work locally across Hyndburn Central (Accrington BB5) — variety without long commutes.

Play a central role in coordinating patient care across Hyndburn Central PCN (based in Accrington, BB5). This part‑time position (18 hours a week) is a non‑clinical Care Coordinator post offering wide‑ranging administrative support across several local practices. You’ll organise meetings, manage correspondence and documents, take minutes, help run the Enhanced Access service, act as a first point of contact for patients and families, and support MDTs, care homes and vaccination campaigns. The role involves using practice clinical systems (EMIS), data to identify patient cohorts and working closely with GPs, pharmacists and other primary care colleagues to improve continuity of care.
This fixed‑term appointment is for two years with the potential to extend or become permanent, and requires flexibility to support different sites across the Hyndburn Central network. It suits organised, compassionate people — particularly those with administration experience, care background or a desire to return to work part‑time.
