The Hospital of St John and St Elizabeth is expanding its Hospice at Home service into Hammersmith & Fulham and is seeking compassionate Health Care Assistants to deliver specialist palliative and respite care to patients who wish to remain at home. This role involves lone-working in patients’ homes, providing personal care and toileting, monitoring skin integrity, supporting nutrition and hydration, prompting medication (including supervising when devices such as syringe drivers are in use), and offering emotional support to patients and their loved ones. You will be expected to recognise changes in symptoms and escalate concerns to the Hospice@Home Clinical Support Nurses or Nurse Manager, keep patients safe and comfortable at end of life, and work collaboratively with other health and social care professionals. The post is permanent, full-time, and subject to a Disclosure and Barring Service check; an NVQ Level 2 or 3 in Health Care and at least 12 months’ experience caring for people with life-limiting illness are desirable.
