Job summary
We are a dynamic PCN based in Nottingham City, comprising of 3 member practices, with approximately 34,000 patients. We are committed to providing excellent care for our patients, as well as support for our staff.We are excited to be able to offer this opportunity for a PCN Care Coordinator (Safeguarding Support) to work closely with practice managers, safeguarding leads, clinicians, and external safeguarding agencies to ensure safe, effective, and consistent safeguarding processes are embedded within the PCN.The post holder will also support PCN service delivery, project work, and personalised care in line with the NHS Long Term Plan and PCN Directed Enhanced Service requirements in our new, on-the-day, minor illness hub based at the same building as one of the member practices. Patients with minor illnesses needing an on-the-day, face-to-face assessment, will be booked in by the member practices according to agreed protocols.This

Main duties of the job
The PCN Care Coordinator (Safeguarding Support) will provide coordination, administrative and safeguarding support to the Primary Care Network (PCN) . The role combines the functions of a Care Co-ordinator with dedicated responsibilities for safeguarding children and vulnerable adults.The role requires excellent organisational skills, strong safeguarding awareness, and the ability to work both autonomously and as part of a multidisciplinary team. Must be able to work on the days that the three practices held there safeguarding MDT meetings .
