Support people with long-term conditions as a PCN QOF Care Coordinator at Stubbington Medical Practice, part of Coastal Fareham & Gosport PCN in Fareham (PO14). In this hands-on role you will organise and manage a caseload of patients, recall and invite them for annual reviews, guide them through care pathways and help them access health, social care and wellbeing services tailored to their needs. Day-to-day tasks include creating and running clinical searches (EMIS), producing letter and text templates, collating and uploading patient data, safety‑netting follow-ups, and working closely with GPs, nurses, reception, pharmacy and PCN services such as social prescribers, home visiting teams and wellbeing coaches. The post demands excellent communication, administrative confidence, sensitivity with patients, and the ability to prioritise and present information clearly.

This permanent, full‑time role (£26,143.20–£26,319.18) is ideal for experienced carers or administrators who value collaborative, patient‑centred practice and ongoing professional development.
