This 30‑hour Health and Social Care Co‑ordinator role with Torbay and South Devon NHS Foundation Trust is based in Torquay (TQ1) and works across the HSCC team as a single point of contact for community health and social care referrals. Hours cover Monday–Friday between 08:00 and 18:00 with some weekend and bank‑holiday working; the vacancy is advertised as a six‑month fixed‑term, band 4 post on Agenda for Change (salary dependent on experience) with the opportunity to progress to band 5 once NVQ Level 4 (or equivalent) is completed — enrolment for that qualification is available within the role. Day‑to‑day you will take calls and enquiries by phone, email and face‑to‑face, collate detailed information, apply Care Act eligibility, identify risk and priorities, carry out Care Act assessments, formulate support plans, commission services where needed and act as a keyworker coordinating a small caseload alongside a multidisciplinary team. The role requires confident IT skills, clear written records, tact and empathy when liaising with service users, families and professionals, and adherence to GDPR and safeguarding processes.

This position will suit carers and compassionate communicators — particularly those balancing family or caring responsibilities who want meaningful, secure part‑time work that supports progression and training. You’ll need recent experience in a health or social care setting, the ability to multi‑task and reprioritise under pressure, and a commitment to anti‑discriminatory practice; a Disclosure and Barring Service check is required.
