Work with Shropshire Community Health NHS Trust in Telford as a UCR Healthcare Support Worker within the Urgent Community Response & Virtual Wards Service. You will provide practical, hands-on care to patients experiencing an acute change in their health, offering a safe, home-based alternative to hospital admission through a mix of face-to-face visits and digital support. The role focuses on re-ablement to maximise patients’ independence and involves basic nursing tasks such as simple dressings, monitoring blood sugars and supporting insulin administration (or training will be provided), with venepuncture competence desirable or supported. You’ll assess, plan, implement and evaluate care alongside nurses and therapists, building strong working relationships across the local health and social care system; operating hours are between 08.00 and 00.00 across seven days, so flexibility across shifts is required.

This permanent Band 3 post (Agenda for Change) offers full-time, part-time or flexible working, salary £24,937–£26,598 pro rata, and is based at Vickers House, Telford (TF7). Essential requirements include NVQ Level 3 in Direct Care (or equivalent), GCSEs in English and Maths (or equivalent), demonstrable healthcare experience and a satisfactory DBS check; additional training will be supported.
