As a Telehealth Care and Support Worker with East London NHS Foundation Trust based at East Ham (E7), you will combine remote vital‑sign monitoring with hands‑on community care to support patients at home and help avoid hospital admissions. Working under the direction of the Clinical Lead and health professionals, you will review telehealth data, follow care plans, escalate any deterioration to the community matron or GP and carry out visits when required — including medication support such as insulin and eye‑drop administration. The role involves close teamwork with urgent care response teams, district nurses, occupational therapists, physiotherapists and carers, and includes weekend shifts as needed. This fixed‑term post (6 months) is offered on Agenda for Change Band 4, with a salary of £31,081–£33,665 and both full‑ and part‑time hours available.

This post suits experienced carers (NVQ3 or equivalent) who are confident working autonomously and building respectful, culturally aware relationships with service users and families. You’ll need good literacy, basic IT skills for telehealth systems, and a willingness to keep accurate clinical records; a Disclosure and Barring Service check is required and skilled‑worker sponsorship applications will be considered.
