Aintree Park Group Practice in north Liverpool is recruiting a PCN Care Coordinator to work across its Moss Lane (Orrell Park) and Oriel Drive (Aintree) sites. This permanent, full-time post is 37.5 hours per week (Monday–Friday within 7.30am–8pm with a 30-minute unpaid break), band 4 pay of £13.81–£14.63 per hour (depending on experience) and 25 days annual leave. The role focuses on proactive patient cohort work to support personalised care, drive uptake of national screening and immunisation programmes, meet Learning Disability and Cancer targets, and coordinate care across health and social services alongside social prescribing link workers to improve outcomes across the PCN.

Ideal for candidates with NHS, social care or general practice experience, the post requires GCSEs including English and Maths, NVQ Level 2 in Business Administration (or equivalent experience), database and data‑collection skills, strong communication and collaborative ability, and the confidence to work on your own initiative under pressure. A DBS check will be required.
