This is a rewarding opportunity to support patients and carers throughout their treatment journey and beyond, helping them navigate care pathways and empowering them to self-manage with confidence.Initially this job will be based at Basingstoke, but there will also be a requirement to travel to Winchester

Main duties of the job
Act as a key point of contact for patients, especially those on Open Access Follow-Up (OAFU).Deliver Level 1 psychological support and perform Holistic Needs Assessments.Provide telephone triage and advice within defined protocols.Support the planning and delivery of Health and Wellbeing programmes.Collaborate with Clinical Nurse Specialists (CNSs) and other healthcare professionals.Ensure timely and accurate documentation and communication.Help patients access appropriate resources and support services.
