Job summary
TheCare Coordinator role is seen as a critical and evolving post to support multi-disciplinaryworking across both Total Health Excellence East & West Primary CareNetworks (PCN) as part of the Integrated Neighbourhood Care Team (INT) to deliver effective,co-ordinated care for adults, particularly those at high risk of a hospitalemergency admission, ED attendances or out of hours care. Proactivelycoordinate personalised care and support planning for the most vulnerablepeople in the community, including the frail/elderly and those living withother long-term health conditions. To undertake some clinical interventionscommensurate with a health care assistant.
Main duties of the job
Tomeet with people, families and carers (in practice, in their home and in othercommunity settings) to co-ordinate their care, review their needs and help themaccess the services and support they require, assisting them to understand andmanage their own health and wellbeing referring to other professionals whereappropriate.Actas a central point of contact to ensure that patients receive the best possiblecare, and the person is supported to achieve the outcomes that are important tothem. This will be achieved by bringing together all the information about apersons identified care and support needs and exploring options to meet thesewithin a single personalised care and support plan, based on what matters tothe person and presenting this at multidisciplinary team meetings (MDT).Tosupport people in preparing for or following-up clinical conversations theyhave with primary care professionals to enable them to be actively involved inmanaging their care and supported to make choices that are right for them. Youwill use knowledge of health and social and other support services available inthe area, including those offered by the community and voluntary sector, tolink people up with these and help them overcome any barriers they mightencounter. The aim is to help people improve their quality of life and avoidunplanned hospital admissions.
