Lead a compassionate, hands‑on team as PCN Personalised Care Team Lead for Rockingham Forest PCN, working across Lakeside surgeries in Corby, Kettering and Oundle. This permanent, part‑time role (30 hours per week) puts you at the heart of personalised care — supervising Social Prescribing Link Workers, Health & Wellbeing Coaches and Care Coordinators, while staying directly involved with patients. You’ll work closely with GPs, community partners and voluntary organisations to help people manage long‑term conditions, loneliness or complex social needs using the What Matters To Me approach. The role is salaried pro rata (£22,838.63–£24,950.42) and sits within Lakeside Healthcare Group, a large, values‑led NHS partner serving thousands across the East Midlands.
You’ll provide day‑to‑day leadership, staff development and induction for new personalised care roles, and collaborate with the Digital & Transformation Manager to improve services and pathways. The ideal candidate will have NVQ Level 3 or equivalent, supervisory experience or leadership training, and experience in NHS or social care; motivational coaching experience and familiarity with systems such as SystmOne are desirable.
