The purpose of this role is to work as a qualified Health Care Assistant, providing qualityclinical and administrative support in an efficient manner to patientsYou will be expected to travel within Tower Hamlets, Newham andWaltham Forest and Havering and so preferable that you are able to drive or ride a motobike
Main duties of the job
Qualified Health Care Assistant to travel across different boroughsTo provide a professional service to our multilingual diverse practice population using available resources to aid effective communicationWork closely with GP/PCN Care Coordinators / GP Assistants to identify patients who have been lost to follow up, and are not being served by the current diabetes service or SMI register. This includes patients who are of working age, LD, vulnerable, housebound, complex care requiring MDT approach, language barriers, concurrent mental health problems etcWrite back/discharge to GP, unless updating EMIS directlyLiaise with participating practices to book patients for reviewsPoint of Care TestingReview patient goalsComplete appropriate onward referrals e.g., to foot health, structured education, weight management programmes etc.Provide lifestyle advice including smoking cessation, weight management, diet and exerciseSupport patients in ongoing referrals/signposting to other services e.g., dietitian service, gym, walking groups etcConsider the mental health and wellbeing of patients and refer on to social prescribers/ wellbeing coaches/ Talking Therapies as appropriateWork with social prescribers, care navigators, Diabetes UK and other health and social care professionalsChaperoning dutiesUndertake other clinical activities
