Job summary

Then this opportunity is for you.Our bedded units are located within Skylark, Henderson and South Bristol Community Hospital and have built up a reputation for delivering excellent patient centred care and enthusiastic teamwork.As a Healthcare Support worker you will assist with the direct care of patients delegated to and supervised by a registered nurse.Your role will provide support to the nursing team, collaboratively with the multidisciplinary team and contribute positively to the delivery of high-quality, compassionate care centred around the core principles of recovery and rehabilitation and the patients wishes.The service supports patients to recover and regain their independence following admission to an acute hospital and the team consists of registered nurses, therapists, advanced clinical practitioners, General Practitioners and a range on unregistered support workers.Our service operates 24 hours a day, 7 days a week.

Main duties of the job
To assist registered nurses and healthcare professionals in the delivery of individualised holistic care which is delivered with compassion, dignity and respect.Work with the multidisciplinary team and contribute to the prevention of harm by participating risk assessments such as nutrition, pressure area care, continence and falls.To assist patient with their care and nutritional support and needs.To safely and accurately record physiological observations such as blood pressure, pulse, respirations and temperatures and escalate and report to the registered nurse accountable for the patients care.To report back any relevant care about the patient to the registered nurse.Assist patients to mobilise safely using equipment and techniques prescribed and recorded in their plan of care.Promote a culture of ambulatory care by supporting patients to be as independent as possible with washing, dressing in own clothes and encouraging mobility from admission.Assist in the provision of prescribed programmes of care e.g rehabilitation and therapy goals under supervision and guidance.Support patient centred rehabilitation to facilitate safe and timely discharges and which reflects individual choice and maximising independence.
