Job Purpose:
The post is responsible for providing professional expertise to the outcome of the business processes for the Long Term Conditions and Prevention programmes.
The post holder will define current processes, facilitate discussions and advise colleagues as to how best practice might be adopted in the definition of future processes, document those processes as they are agreed and oversee their delivery.
The post holder will have senior manager responsibility within Birmingham and Solihull ICS and will be a subject matter expert leading strategically and operationally on the major area of activity. You will be responsible for deputising for the Head of LTC and Prevention as required, expanding on knowledge, skills and experience within professional development.
As a Senior Programme Manager in the Long Term Conditions and Prevention team, the post holder will work as part of a dynamic team and will lead one or more programme areas below. The Long Term Conditions (LTC) Programme includes - Circulatory Diseases, CVD Prevention, Stroke, Diabetes, Respiratory, Long COVID, Frailty, Dementia, and End of Life. The Prevention Programme includes NHS Long Term Plan Prevention Priorities including Alcohol, Tobacco, Obesity, TB, & HIV Screening.
This post holder will primarily lead the Respiratory portfolio including Pulmonary Rehab, Home Oxygen Service, Community Diagnostics, Long Covid, LTBI, TB Service, Winter Planning, and Clinical Pathways Development and Integration.
The post holder will have senior manager responsibility within Birmingham and Solihull and will be a subject matter expert leading strategically and operationally on the major area of activity. You will be responsible for deputising for the Head of LTC and Prevention as required, expanding on knowledge, skills and experience within personal and professional development.
As a Senior Programme Manager in the Long Term Conditions and Prevention team, the post holder will work as part of a dynamic team and will be leading one or more programme areas including Cardiovascular Disease, Stroke, Diabetes, Respiratory, Frailty, and Prevention (Tobacco, Alcohol, Obesity, TB, HIV Screening, Infectious Diseases) work programmes in the ICB.
The LTC and Prevention programme will work closely with Health Inequalities and other ICS programmes to ensure alignment and effective delivery of priorities to develop collaboration and joint working throughout the system. The programme is committed to a wide-ranging portfolio of transformation programmes which draw on the new partnership arrangements within the ICS for delivery.
