PCN Care Coordinator — Fareham & Portchester PCN, based in Fareham (PO15). This part‑time, permanent role (15 hours per week) is ideal for caring professionals or those with lived experience who need a role that fits around other responsibilities — one shift will cover a Friday afternoon and the remaining hours are negotiable. You will work across four practices serving around 47,500 patients as part of a multi‑disciplinary team of GPs, nurses, paramedics, pharmacists, social prescribers and physiotherapists, with strong in‑house supervision, a focus on staff wellbeing and an inclusive culture that values diversity. The salary band is £9,555–£9,906 pa (pro rata), EMIS and primary care experience are desirable, and the post is subject to a DBS check.

In this hands‑on role you will proactively identify and manage a caseload using population health data to deliver personalised care and single personalised care and support plans (PCSPs). Day‑to‑day tasks include supporting shared decision‑making, answering queries and arranging appointments, coordinating care across health and social services, signposting to self‑management courses and employment or benefits support, referring to social prescribing link workers, helping people access personal health budgets and assisting with long‑term condition reviews, online consultation processing and blood pressure result management.
