As a Patient Flow Coordinator at City Health Care Partnership CIC based at Rossmore Community Rehabilitation Centre in Hull (HU3), you will organise and coordinate safe, effective discharges for people with physical and complex needs. You’ll work closely with Occupational Therapists, Physiotherapists and other members of a multi‑disciplinary therapy team, supporting transfers of care across community and hospital settings and contributing to rehabilitation plans under the guidance of registered practitioners. This permanent, full‑time role is 37.5 hours per week (Monday–Friday), paid at Agenda for Change Band 4 (£27,485–£30,162), and requires strong communication, high IT literacy, and the ability to work well under pressure.

This opportunity is ideal for experienced carers and compassionate women seeking a stable, respected clinical role where development matters — an NVQ4 or Foundation Degree in Health and Social Care or demonstrable rehabilitation experience is essential, together with knowledge of safeguarding, the Mental Capacity Act and safe discharge processes.
