Job responsibilities
The Senior Health Care Assistant plays a key role in our Community Services team. This service includesthe Hospice at Home team, the Wellbeing Programme, and the Rapid Response service, which is delivered in partnership with South Warwickshire NHSFoundation Trust.This role will primarily be focused onsupporting patients within the Hospice at Home and Wellbeing Programme, workingacross a 7-day rota between the hours of 8:00am and 8:00pm. On occasion,the postholder will be required to work within the Rapid Response team, whichis a 24-hour service and will include internal rotation onto night shifts from8:00pm to 8:00am.When working within the Hospice at Home team,the Senior Health Care Assistant will provide high-quality palliative and end-of-life care topatients and their families in their own homes. This care involves supportingpatients with personal care, emotional wellbeing, and practical needs. The role will also include delivering short periods of respite care, offering essentialsupport and rest to family members and carers during what is often anemotionally challenging time.A key part of the role involves closecollaboration with colleagues from the NHS, social services, local care homes,and voluntary sector organisations to ensure that each patient'spreferences, particularly the wish to remain and die at home are fully supportedand respected.Within the Wellbeing Programme, the Senior Health Care Assistant will work as part of a multidisciplinary team to enhance the quality of lifefor patients and those close to them within the hospice setting.
