Job summary
An exciting opportunity has arisen for a patient flow coordinator to join our fast paced intermediate care team unit in Hull.The post holder will support the MDT in planning and arranging safe, effective discharges for patients. They will work under the guidance of registered practitioners with a diverse client group and will be joining an established multi disciplinary team working at Rossmore Community rehabilitation bedded areas.The role requires you to work with both Occupational Therapists and Physiotherapists as well as independently under the guidance of a Therapist.The post involves working with people with physical problems within the community or hospital settings You should be empathetic, have excellent communication and listening skills and the ability to support and motivate others to achieve their potential.
